Empowering Choices: The Essence of Informed Consent in Birth

Birth is a miraculous journey that ushers in new life, shaping families and communities. Throughout history, the act of giving birth has evolved considerably, and so has the concept of informed consent. Informed consent in birth is a fundamental principle that aims to respect a person’s autonomy, ensuring they are equipped with the knowledge necessary to make well-informed decisions about their birthing experience. But….we’re not so great about achieving this in the Western world. In fact….we’re really horrible at informed consent (and by ‘we’ I mean you….. talking to you, medical establishment!)

At its core, informed consent is about ensuring expectant parents have access to comprehensive information about available birth options, potential risks, and benefits associated with each approach. This process fosters communication and collaboration between healthcare providers and parents-to-be. It enables individuals to take an active role in their birthing journey, leading to a sense of empowerment and ownership over their choices. Unfortunately, I have an entire team of doulas with COUNTLESS stories about how parents wishes are NOT respected, their bodies are not respected and frankly, there’s a whole lot of abuse of power out there. It’s gross….

Informed consent starts during prenatal care, with open and honest discussions about various birthing methods, such as natural birth, cesarean section, water birth, or the use of pain relief options. Providers should also explain potential interventions that might be suggested (or required) during labor and the reasons behind them. It is crucial for parents to understand the implications of each decision, both for themselves and their baby.

Respecting informed consent is particularly significant during emergency situations or unplanned interventions. In these scenarios, healthcare providers must communicate clearly, offer available options, and obtain consent before proceeding whenever possible. If an individual is unable to provide consent due to medical reasons, the healthcare team should involve a designated decision-maker, if one has been designated in advance.

Informed consent in birth goes beyond merely presenting information; it is about creating a compassionate and supportive environment where parents feel heard and respected. Listening to their concerns, fears, and preferences fosters trust and strengthens the doctor-patient relationship.

When the principles of informed consent are upheld, parents are more likely to feel satisfied with their birthing experience, regardless of the outcome. In contrast, disregarding this principle may lead to feelings of helplessness, frustration, and even trauma. This is reason #1 why we have so many suffering from PTSD post birth. It’s not just because a human came out of the laboring person….it’s mostly because of how they were treated in labor and delivery.

Informed consent is the cornerstone of respectful and patient-centered maternity care. By providing expectant parents with comprehensive information, respecting their autonomy, and involving them in decision-making, we can empower individuals to make choices that align with their values and preferences. A well-informed and supported birth experience paves the way for a positive start to parenthood and enhances the physical and emotional well-being of both parents and their newborn. As healthcare providers and communities, it is our collective responsibility to champion informed consent and promote a culture of respect and empowerment in birth. And doulas out there…..we can help by encouraging our clients to find THEIR own voices during labor. Not yours.
